Athletic Clearance!

For the 18-19 school year, all athletics physicals will be submitted online via https://athleticclearance.fhsaahome.org/index.php.

If the physical is dated before June of 2018, they will need to get a new physical to ensure that they will be eligible for the entire season. POP WARNER physicals and the health department SCHOOL ENTRY physicals are NOT ACCEPTED. Each athlete is required to have health insurance; if you do not have insurance you may purchase school insurance. Please see the front office for more information about school insurance or visit www.schoolinsuranceofflorida.com.

Step one: Visit the link above and register to create your account. (If you have used Athletic Clearance in the past for a high school student you do NOT have to create a new account)

Step two: After you have created an account and have logged in, click the “Start Clearance Here!” button to get started.

Step three: Select the 18-19 school year, Tavares Middle as your school, and the sport your student is interested in. Then click SUBMIT. (Note: Select one sport for now. If your student is interested in multiple sports you will be able to add more to your clearance later in the process)

Step four: Complete all required student information, then click SAVE. (Note: If your student does not have an email/cell phone number, or if you are not comfortable providing that information, please complete those sections with your own email and cell phone number)

Step five: Upload the required documents as noted in the example photo, then click SAVE. The Athletic Clearance site will save your progress if you need to print the forms and upload at a later time.

Step six: Complete the Medical history section by selecting YES or NO on each question. If you select YES for the “History of skin conditions” or “Additional comments” sections, the interface will provide you a space to type in comments. When finished, click SAVE.

Step seven: Complete the required Parent/Guardian information, then click SUBMIT.

Step eight: Type your name to digitally sign each of the required documents. There are sections for parent and student signatures. Then click SUBMIT.

Step nine: If you would like to make a donation to Tavares Middle School athletics, you may enter any amount you’d like in the space provided. It is NOT required. You can also click the SKIP TO CONFIRMATION button to proceed without donating.

Step ten: Print the final confirmation page, then sign and date. Turn this page in to the main office to verify that you have completed the online clearance process. Verify your digital signatures, then click SUBMIT. (Note: If your student may be interested in multiple sports, you can indicate them on this page. You can revist your clearances and add more sports later if your are unsure at the end of this process)

Final step: You will be returned to the “Clearances” page were you will be able to view the status of your submission. You will begin with an UNCLEARED indicator. Once your documents have been reviewed and approved your status will change to a CLEARED indicator. Once you have been CLEARED, then your student is all set to participate in TMS athletics!
